Fix emoji paths in relative root configurations
## What does this MR do?
If a site specifies a relative URL root, emoji files would omit the path from the URL, leading to lots of 404s.
## Are there points in the code the reviewer needs to double check?
At first, I tried to use `ActionView::Helpers::AssetUrlHelper.asset_url` since this is what it's intended to do. But this helper function is extremely slow, and it took minutes to generate the URLs for the hundreds of links needed for each emoji.

Because emojis were broken in relative URL installations

Fixing problems with events for import/export
Fixes https://gitlab.com/gitlab-org/gitlab-ce/issues/19202
A couple of issues related to target being missing in exported `Events` (as being polymorphic and not have `ActiveRecord` relationships is a bit more tricky than normal models) plus as the export was in JSON, the import retrieves hashed fields as stringified hashes and not symbolized - so fixed that as well, which was the cause of https://gitlab.com/gitlab-org/gitlab-ce/issues/19202

Import/Export Version has been bumped to 0.1.1 as theses changes to events won't work very well with old exports - forcing users to generate a new export in the new version.

Fix restore warning message
## What does this MR do?
Fix the restore Rake task so it properly outputs the database warning. This is a pretty important warning and it was not even being output. After this fix, the output looks like the screenshot below.

Fix privilege escalation issue with OAuth external users

This MR fixes a privilege escalation issue, where manually set external users would be reverted back to internal users if they logged in via OAuth and that provider was not in the `external_providers` list.

Better debugging for memory killer middleware
This adds more info to the warning messages output by `MemoryKiller`.

Use clock_gettime for all performance timestamps
This MR adjusts the performance monitoring code to use `Process.clock_gettime` (thus `clock_gettime(3)`) instead of `Time.now`.
Using `Time.now` / `Time.new` adds more overhead than `Process.clock_gettime`, it also doesn't provide a way of getting timestamps in nanoseconds (which `Process.clock_gettime` does allow).

Fix temp file being deleted after the request while importing a GitLab project
Fixes https://gitlab.com/gitlab-com/infrastructure/issues/151
In production, the temporary uploaded file is getting deleted straight after the request so the Sidekiq worker is unable to find it in `/tmp`
Also, improved erroring/logging of this situation.

Fix RangeError exceptions when referring to issues or merge requests outside of max database values
When using #XYZ in Markdown text, if XYZ exceeds the maximum value of a signed 32-bit integer, we get an exception when the Markdown render attempts to run `where(iids: XYZ)`. Introduce a method that will throw out out-of-bounds values.

Track method call times/counts as a single metric
This changes method call tracking so only a single metric is emitted regardless of the number of calls. This allows us to more accurately measure the total execution time of a method as well as the number of times a method is called. See 851e3ff7578973c2206628424eac3b951a3c656d for more details.
Method call tracking tracked calls individually meaning the end statistics may not always be accurate enough to get a good understanding of where time is spent.

Allow customising of queries used for `update_column_in_batches`
This MR makes two changes to `add_column_with_default` and `update_column_in_batches`:
1. `add_column_with_default` no longer wraps the entire set of updates in a single transaction, preventing any locks from sticking around for the duration of the entire transaction
2. `update_column_in_batches` now takes a block which can be used to customise the queries. This uses Arel as messing with raw SQL strings is a total pain
In !4381 there's a need for updating existing rows/columns in a table in batches using a custom `WHERE` condition. Without the changes in this MR this would not be possible.

Reduce SQL query counts in IssueReferenceFilter
## What does this MR do?
This MR adds a preparation phase for reference filters that allows them to prepare/create data structures used while iterating over HTML nodes. In this particular case the preparation phase is used for issue references to greatly cut down the amount of queries executed to get projects/issues for Markdown references.

## Why was this MR needed?
Rendering Markdown containing issue references would run at most two queries for every issue reference: one to get the project and one to get the issue from said project. When rendering Markdown with lots of issue references this would result in _a lot_ of queries being executed.

Eager load project relations in IssueParser
## What does this MR do?
This changes the ReferenceParser class to eager load various associations. This in turn results in the permissions checking code (e.g. the `Ability` model) to _not_ run dozens if not hundreds of extra SQL queries depending on the amount of references involved (in a single document).

## Why was this MR needed?
In !4410 it was revealed a _lot_ of a queries came from the `Ability` model and the code it would call. In many cases this was because the code would simply get a project, then get the owners; or get a group, then get some association of that. Eager loading these associations is a fairly simple solution and greatly cuts down the number of queries.

Add environments and deployments
This MR is a continuation of https://gitlab.com/gitlab-org/gitlab-ce/issues/17009.
The current implementation is as follow:
1. We have two new tables: `environments` and `deployments`.
2. We have a new tab: `Environments` under `Pipelines` where you can see all you environments and add a new one.
3. We add a new option to `.gitlab-ci.yml` to track where we should create a deployment for environment.
4. If environment in `.gitlab-ci.yml` is specified it will create a deployment. **If environment does not exist it will be created.** (this got changed)
5. The deployment is always successful and shows the time of the action, in that case a build that presumably should do deployment. In the future we could extend deployment with statuses: success, failure. We could extend deployments with information that this is partial or full deployment.
6. User have to create environments that he will track first.
7. User can remove environments.
8. User can retry/rollback past deployment (in that case we retry past build). The new build when succeeds it will create a new deployment.
9. Currently environment have only one parameter: `name`. In the future it should have: `variables`, `credentials` and possibly `runners` and maybe other resources.
10. Currently deployment have this parameters: `sha`, `ref`, `deployable (in this case a build)`, `user (who triggered a deployment)`, `created_at`.
The `.gitlab-ci.yml`:

By eager loading these associations we can greatly cut down the number
of SQL queries executed when processing documents with lots of
references, especially in cases where there are references belonging to
the same project.
Since these associations are so specific to the reference parsing
process and the permissions checking process that follows it I opted to
include them directly in IssueParser instead of using something like a
scope. Once we have a need for it we can move this code to a scope or
method.
